- Form W-4: You are welcome to complete and submit a new W-4 form to us at any time. Our employment forms are located online at http://www.sdistaffing.com/employees/employment-documents/.
- 2012 W-2: Your tax form will be sent to you electronically via email as soon as possible after the first of the year. If you are unable to download and print your form by 01/30/12, a hard copy will be mailed to the address we have on file no later than January 31, 2013.
